Securities Donation

Donating securities, including stocks, bonds, and mutual funds is an efficient and easy way to support SickKids today. Simply fill out the online form below, including your broker’s contact information so that they will receive a copy of the form and work with you to initiate the transaction.

Complete all required sections of the transfer form below including the ticker number of the assets being transferred (which can be provided by your broker and/or investment advisor, or listed on your statements) and your broker’s contact information.
After submitting the form, an email confirmation will be sent to both you and your broker and/or investment advisor with a PDF attachment outlining the required information and next steps. After discussing with your broker and/or investment advisor, they will transfer the shares to The Hospital for Sick Children Foundation. If you are a self-directed investor, please refer to your own direct investing platform and follow the necessary steps to initiate the transfer.
SickKids Foundation’s broker will work with your broker and/or investment advisor to complete the transfer process. Please be advised that the value of the donation receipt will be based on the closing price of the security, on the day the securities are received by the Foundation’s broker. Once proceeds are received, we will issue your charitable tax receipt, if applicable.
